   Magistrate Court Forms (Separate Website)
These forms have been developed by the Uniform Forms Committee of the Council of Magistrate Court Judges. They are not mandatory forms for filings in the Magistrate Courts. All Magistrate Courts in Georgia are encouraged to accept pleadings filed using these forms.

Official Forms for the United States Bankruptcy Courts (Separate Website)
Federal courts have exclusive jurisdiction over bankruptcy cases. Bankruptcy cases cannot be filed in state court. Each of the 94 federal judicial districts handles bankruptcy matters. The primary purposes of the law of bankruptcy are: (1) to give an honest debtor a "fresh start" in life by relieving the debtor of most debts, and (2) to repay creditors in an orderly manner to the extent that the debtor has property available for payment. This web site contains the official forms for the United States Bankruptcy Courts. Official Bankruptcy Forms must be used to file and take action in bankruptcy cases. Procedural Forms also may be necessary for use during the course of some bankruptcy proceedings. Additional filing instructions, along with the forms, are available in the Bankruptcy Forms Manual (PDF format).
