  Family Violence Self-Help Manual      This manual can help you learn how to use the law to help you and your family members escape from domestic violence.  This manual contains legal forms and explanations of the law.  In this Manual, you will find support information, family violence agencies and their contact information and important legal forms, like these 3 important forms:

• Petition for Temporary Protective Order
• Family Violence Ex-parte Protective Order
• Family Violence 12-Month Protective Order
• Financial Affidavit
• Child Support Worksheet and Schedules
• Child Support Addendum (to be added to the 12 Month Protective Order)
• Income Deduction Order (includes instructions to the employer)

  Stalking Self-Help Manual    In this Manual contains legal forms and explains the stalking law. It also includes  these 3 important forms:

• Petition for Stalking Temporary Protective Order
• Stalking Ex-parte Temporary Protective Order
• Stalking 12-Month Protective Order  

  If you already have a protective order, here are some forms that might be helpful to you:

• Order to Modify Prior Protective Order
• Order for Continuance of Hearing and Ex-parte Protective Order
• Dismissal of Temporary Protectiver Order

Long Term Protection. If you have  a 12-Month Order (or less), you may ask the court to extend the order to last three years or to make it permanent. This must be done before your TPO expires. Here are the forms you need to extend a TPO:
